The following are the Virginia school admission requirements related to physical examinations and immunizations for students. New students and transfer students are required by law to submit documentation prior to admission that these requirements have been met.
Commonwealth of Virginia law requires that all children entering Virginia schools must provide documentation of the required immunizations. Such documentation must be on an official health department or physician office form and must include the month/day/year the immunizations were administered. State Board of Health regulations mandate the exclusion, by the school's admitting official, of any student who is not in compliance with immunization requirements unless they have been granted either a religious or medical exemption, or have had at least one dose of the required immunizations and have a schedule to complete the required vaccinations.
A copy of a physical examination is also required for any student in grades K-5 prior to admission for the first time to school in Virginia. It must be performed by a Licensed Physician, Licensed Nurse Practitioner, or Licensed Physician Assistant within 365 days prior to the date of entering school. For transfer students, records showing that a physical examination by a Licensed Physician, Licensed Nurse Practitioner, or Licensed Physician Assistant was completed prior to enrolling in another school and a copy of the report of such examination, or telephone verification from the other school with records to follow, may be provided. Regulations mandate the exclusion by the school's admitting official of any student who fails to provide a copy of the report of such examination.
